Before You Print

  • Not legal advice. This is a general form. For your own situation, consult a lawyer, especially if your case is unusual or a lot of money is involved.
  • It does not need a notary to be valid. Sign it, and keep a signed copy.
  • Under DOLE Labor Advisory No. 06, Series of 2020, an employer must give a Certificate of Employment within 3 days of the employee's request. A former employee can ask for one too.
  • A Certificate of Employment states the facts of the work: the dates and the kind of work done. Add pay or other details only if the employee asks for them, for example for a loan or visa application.
  • Print it on the company letterhead if you have one. It does not need to be notarized. An embassy or foreign employer may ask for it to be notarized and apostilled. See the guide Apostille.
