Before You Print
- Not legal advice. This is a general form. For your own situation, consult a lawyer, especially if your case is unusual or a lot of money is involved.
- It does not need a notary to be valid. Sign it, and keep a signed copy.
- Under the Local Government Code (Republic Act No. 7160), you may complain to the Punong Barangay orally or in writing, and pay the barangay's filing fee. Many barangays have their own printed complaint form. You can bring this as your draft, or copy it onto their form.
- File it in the barangay where the person you are complaining against lives. For disputes over land, the barangay where the land is may be the right place. Ask the barangay secretary.
- Not every dispute goes to the barangay. For example, cases involving the government, people who live in different cities or municipalities (unless their barangays are next to each other and they agree), and more serious crimes go straight to the proper office or court. See the guide Barangay conciliation.
- Bring copies of your evidence, such as receipts, photos, messages and the names of your witnesses. Keep a received copy of your complaint.
For the steps around this document, see the guide Barangay conciliation.
