NOTARY PUBLIC

Before You Print

  • Not legal advice. This is a general form. For your own situation, consult a lawyer, especially if your case is unusual or a lot of money is involved.
  • You will swear before the notary that it is true, then sign (this is called a jurat).
  • Do not sign it yet. You sign in front of the notary, with a valid government ID.
  • Many offices and embassies ask for a PSA Certificate of No Marriage Record (CENOMAR) instead of this affidavit, or together with it. Ask the office what it accepts. See the guide Getting a CENOMAR.
  • If you were married before and the marriage ended (by death, annulment, a court ruling that it was void, or a foreign divorce recognized here), you are not "single" in this sense. Your status is widowed, or the court decision is the proof. This form does not fit your case.
  • Only sign it if it is true. Swearing to something you know is false is perjury, a crime under the Revised Penal Code.
