NOTARY PUBLIC

Before You Print

  • Not legal advice. This is a general form. For your own situation, consult a lawyer, especially if your case is unusual or a lot of money is involved.
  • You will swear before the notary that it is true, then sign (this is called a jurat).
  • Do not sign it yet. You sign in front of the notary, with a valid government ID.
  • A child travelling abroad alone, or with someone other than a parent, usually also needs a DSWD Travel Clearance. This affidavit is one of the papers DSWD asks for. It does not replace the clearance.
  • DSWD asks for the consent of both parents, or of the solo parent or legal guardian. For a child born to parents who are not married to each other, the mother usually signs.
  • Parents who are abroad can sign before a Philippine embassy or consulate. DSWD may also ask for a separate Affidavit of Support and other papers. Check the list of papers with the DSWD field office.
