Before You Print
- Not legal advice. This is a general form. For your own situation, consult a lawyer, especially if your case is unusual or a lot of money is involved.
- You will swear before the notary that it is true, then sign (this is called a jurat).
- Do not sign it yet. You sign in front of the notary, with a valid government ID.
- Write only facts you know yourself, in the first person, one fact per line. Lying under oath is perjury, and you can be charged for it.
- Check the other affidavit templates first: a specific form, such as an Affidavit of Loss or of Residency, usually says what the office needs in fewer words.
- Ask the office that will receive the affidavit what it must say. Many offices have their own form.
