Before You Print

  • Not legal advice. This is a general form. For your own situation, consult a lawyer, especially if your case is unusual or a lot of money is involved.
  • It does not need a notary to be valid. Sign it, and keep a signed copy.
  • SSS and GSIS have their own forms and rules for representatives and for pensioners who cannot go in person. Check with them before relying on this letter. They may ask for their own form or a notarized Special Power of Attorney (SPA).
  • The same goes for a local government's senior citizen pension or a cash aid program: ask the office, such as the OSCA or the social welfare office, what a representative must bring.
  • This is a simple letter, not notarized. Give your representative a copy of your ID and have them bring their own.
